Changing your address or other details?

Don’t leave your entitlements behind!

If you change your address or other contact details you need to notify:

• The Union
• Cbus, your superannuation scheme
• ACIRT, your redundancy Scheme
• LSPC, your Long Service Leave

If you do not, you may miss out on benefits you are entitled to.

Complete the following form and submit it immediately to protect those benefits.Your information will be simultaneously emailed to the appropriate administrative people in all the above organisations so that your records can be updated.

Note: A return confirmation message will be requested with this form - unless you supply a valid return email address you will not receive confirmation. If you need to change your email address details only, please use this form instead.
